Bone replacement

With the loss of teeth, we have to face several unpleasant consequences in the long term. Not only the migration of the teeth adjacent to the missing tooth and the protrusion of the opposite teeth cause problems, but large bone defects often develop in the place of the lost teeth and the tight gums found next to the typically healthy teeth also disappear over time. Above, in the area of the back chewing teeth, it is typical that after the loss of the teeth, the bone next to the roots of the teeth withers away, is absorbed, and is replaced by an air-filled sinus. When designing an implant, this missing bone height (and width) makes bone replacement necessary. In our surgery, depending on the extent of the deficiency, we perform this with a smaller, so-called internal, or a larger, so-called open sinus lift. For bone replacement, we use bone substitute materials from the world-renowned Swiss company Geistlich, which has been a pioneer in the development of regenerative biomaterials since the 1980s. During the operation, we also try to collect as much of the patient’s own bone as possible, which, mixed with the bone substitute material, turns the replaced area into bone with greater success. In the case of the lower jaw, the lack of teeth has similar consequences. The absence of teeth in the jaw also causes atrophy, and this process can be accelerated even more by wearing removable dentures for a long time. When designing implants, the position is significantly influenced by the nerve running inside the jaw, the track of which must always be taken into consideration during planning. In simpler cases, it is only necessary to increase the thickness of the jaw, in more complex cases, in addition to the thickness, it may also be necessary to increase the height of the bone. For these dental procedures, we use a mixture of the patient’s own bone and bone substitute material, which is attached to the jawbone with the help of a special membrane and small screws. The implants are inserted only after ossification of the area. For our patients who do not want animal-derived or synthetic bone substitute materials to be placed into their bodies, or who are afraid of the bone substitute material being rejected, there is also the option of using only their own bone for the bone replacement: by transforming and shaping the bone block extracted from the region of the lower wisdom teeth, we can achieve the replacement of complicated bone defects with high safety, using only the patient’s own bone and mini fixation screws.
